RESERVATION FORM

Things to remember to include:

• Contact name and phone number
• Event name
• Date and Time -- be sure and note in the correct spot if it will be a one time event or recurring event.   If it is a recurring event, list any dates that will be skipped.
• Location -- list where in the church you need space reserved or list "away" if not held on church property and state where the event will be held.
• Room set-up -- be as specific as possible.
• Childcare, vans, sound support or any other details should be filled in at the bottom of the form.

WORK REQUEST FORM

This form is used if you need a project done by the church staff. (Ex: reminder postcards for your ABF class, flyers for an event)

FACILITIES MAINTENANCE WORK REQUEST FORM

This form is to be used when church facilities need special attention, such as repair, cleaning, spraying, etc.

Once you fill out the form, please turn it in to the church office.
